What the consultation form asks for

The form on the homepage asks for your first name, last name, email, phone number, whether you plan to use private pay or insurance (and the plan name, if you use insurance), and optional additional information.

Sending the form requests a free 15-minute consultation. It does not book an appointment or start a therapist-client relationship. Please don’t include crisis details or a detailed medical history. It is a contact form, not a clinical intake.
